Care Manager RN - Foster Care

Job Description

We are looking for a compassionate and dedicated Care Manager RN to join the Superior HealthPlan team. In this role, you will assess, plan, and coordinate medical services for select members to ensure quality and cost-effective care, focusing on promoting positive outcomes and holistic well-being.

Responsibilities

• Conduct comprehensive assessments of members' health status and treatment plans.
• Develop personalized care plans with specific goals and interventions.
• Collaborate with healthcare providers and community resources to address members' needs.
• Provide education to patients and caregivers.
• Monitor and coordinate ongoing care management services.
• Participate in care management committees and special projects.

Requirements

• Graduate from an Accredited School of Nursing.
• Bachelor's degree in Nursing preferred.
• 2+ years of clinical nursing experience and 1+ years of case management in managed care.
• Current state's RN license.
• Knowledge of utilization management principles.
