Case Manager

Job Description

House of Hope Green Bay, Inc is currently seeking a dedicated Case Manager to join our team. In this role, you will be responsible for providing case management services to clients in our Emergency Shelter and Housing Stability programs. The Case Manager will work closely with interns and volunteers to ensure that clients receive the necessary support to achieve success. Additionally, compliance with legal regulations and grant standards will be a key focus of this position.

Responsibilities

• Provide case management activities, housing navigation, and oversee case plans for assigned clients.
• Collaborate with Advocates and Lead Case Manager to ensure effective service delivery.
• Screen clients for program eligibility and manage client-related databases.
• Deliver crisis intervention and maintain a safe environment in the emergency shelter.
• Participate in program development and community engagement activities.

Requirements

• Valid Wisconsin driver's license and clear criminal background check.
• Degree or relevant work experience in social work or related field.
• Strong interpersonal skills and ability to work with diverse populations.
• Knowledge of local housing programs and community resources preferred.
• Ability to work independently, handle multiple tasks, and adapt to changing situations.
