Job Description
We are looking for a qualified Complex Care Social Worker (LMSW) to join our team at LifeBridge Health. As a full-time member, you will play a crucial role in facilitating discharge planning for difficult-to-place, high-cost, and escalated patients. In collaboration with the interdisciplinary team, you will implement a multi-step escalation protocol to identify and address barriers to efficient discharges within set length-of-stay goals.
