We are currently looking for a passionate and experienced HOME CARE REGISTERED NURSE (RN) to join Liberty Home Care on a Full-Time basis. As a valued member of our team, you will actively participate in the interdisciplinary care planning process and provide professional nursing services in alignment with established care plans. Your dedication will help promote favorable outcomes through collaborative practice patterns and resource utilization.

Responsibilities

• Actively participate in interdisciplinary care planning.
• Provide professional nursing services based on established care plans.

• Document services accurately and thoroughly to ensure quality care.
• Demonstrate effective interpersonal relationships and accountability in work assignments.

Requirements

• Completion of nursing education from an approved school.
• Current RN licensure in the state of operation.
• Clinical nursing experience preferably in home health settings.
• Knowledge of medical-surgical nursing and clinical skills.
• Effective communication and computer proficiency.
• Strong problem-solving and critical thinking abilities.
• CPR certification.
