Sunrise Senior Living is looking for compassionate Home Health Aides to join our team. As a Care Manager, you will provide personalized care and assistance to residents in our assisted living and reminiscence neighborhoods, ensuring they lead fulfilling lives.

Responsibilities

• Build meaningful relationships with seniors and their families.
• Assist residents with activities of daily living and individual care needs.
• Participate in developing Individualized Service Plans (ISP) and monthly updates.
• Ensure resident safety, well-being, and satisfaction.

Qualifications
