HOSPICE CLINICAL MANAGER - RN

Job Description

Join Liberty Hospice as a HOSPICE CLINICAL MANAGER - RN and make a difference in the lives of our patients and families. In this full-time role in Cumberland County, you will play a vital role in coordinating patient services, supervising care staff, and providing exceptional clinical management.

Responsibilities

• Coordinate case management for all patient services.
• Document patient care and staff performance accurately.
• Supervise assigned care staff and support the Administrator as needed.
• Provide direct patient care, including case management, Oasis care planning, and efficient staffing utilization.

Requirements

• Current RN licensure in North Carolina.
• Minimum of three (3) years of clinical nursing experience, hospice experience preferred.
• In-depth knowledge of hospice/homecare regulations.
• Strong organizational and case management skills.
• Excellent communication and critical thinking abilities.
• Familiarity with computer applications and CPR certification.
