Summary
The Inpatient Case Manager, working in collaboration with the clinical team and medical provider, is responsible for discharge planning coordination and intervention. The role focuses on promoting patient wellness, improving care outcomes, ensuring efficient health services utilization, and minimizing payment denials among a patient population with complex health needs.
Requirements
- Basic professional knowledge; Bachelor's degree or equivalent;
- BSN required from CCNE accredited schools;
- Experience in Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager is advantageous.
