Curana Health, a provider-led healthcare organization, is seeking a dedicated Nurse Practitioner or Physician Assistant to join our team in Springfield, MA. As a crucial member of our post-acute care setting, you will have the opportunity to deliver direct patient care services and make a meaningful impact on the health and well-being of senior living residents.

Responsibilities

• Conduct clinical interventional and episodic direct care visits, and appropriate re-evaluation of care visits.
• Provide ongoing evaluation and treatment for chronic, co-morbid illnesses.
• Coordinate care with collaborating physicians and other healthcare disciplines.
• Assist in conducting Annual Wellness Visits and other medical evaluations.
• Maintain effective communication with patients, families, and multi-disciplinary teams.

Qualifications

• Valid Nurse Practitioner or Physician Assistant license in the state of operations.
• Current board certification in the respective field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
