Job Description
Gentiva Hospice, a part of the Gentiva family, a leading provider in hospice, palliative, home health, and personal home care, is currently seeking a compassionate and empathetic Registered Nurse to join our team. In this role, you will work under the direct supervision of the Administrator, Executive Director, or Patient Care Manager Senior, providing essential patient care and acting as a crucial link between physicians, patients, and the hospice team.
Responsibilities
- Provide direct patient care services and support to hospice patients and their families.
- Manage an interdisciplinary team to ensure high-quality hospice care delivery.
- Evaluate patient and family needs to develop and implement individualized care plans.
- Maintain accurate and up-to-date patient clinical records.
- Participate in quality assessment and improvement programs.
Requirements
- Current RN license, or ability to obtain one in the state of employment.
- Minimum of 1 year experience in hospice, home health, or oncology nursing.
- Compassionate and empathetic attitude towards patients and families.
