Job Description
Join the Gentiva Hospice team as a Registered Nurse, providing compassionate care and support to patients and their families. As a crucial member of our team, you will ensure the highest level of care for those in need, serving as a liaison between patients, families, and the hospice team.
Responsibilities
- Provide direct patient care services and support to hospice patients and their families.
- Manage an interdisciplinary group to enhance the quality of hospice care.
- Evaluate patient and family needs, and assist in developing and executing individualized care plans.
- Maintain accurate and complete patient clinical records.
- Participate in quality assessment and performance improvement programs.
Requirements
- Compassionate and empathetic towards patients and their families.
- Current RN license or ability to obtain one.
- 1+ years of nursing experience in hospice, home health, or oncology.
