Job Description

The KAREN ANN QUINLAN MEMORIAL FOUNDATION is seeking a dedicated and compassionate Registered Nurse to join our team as an RN Case Manager. This full-time role involves providing essential care and support to patients and their families during challenging times.

Responsibilities

• Coordinate and provide ongoing physical and psychosocial assessment of hospice patients.
• Collaborate with the Interdisciplinary Team (IDT) to address new nursing diagnoses and make necessary changes to care plans.
• Supervise and instruct hospice aides and LPN staff.
• Assist patients and families in obtaining necessary medical equipment and supplies.
• Communicate with physicians and other healthcare personnel to report changes in patients’ condition and identify their needs.
• Administer medications, perform patient/family teaching, and support patient and family independence.
• Complete timely chart documentation and maintain caseload productivity in compliance with agency standards.
• Incorporate the physiological and developmental age of patients into the overall plan for the delivery of patient care.
• Participate in all required in-service education programs and meetings.

Requirements

• Strong nursing assessment and documentation skills.
• Clinical knowledge to support the dying process.
• Excellent interpersonal and communication skills.
• Proficiency in operating a computer and electronic medical record.
• Diploma, Associate, or Bachelor of Science in Nursing; BSN preferred. Licensed to practice as a Registered Nurse in the State of New Jersey; licensure in Pennsylvania if practicing in PA.
• Minimum one-year adult, pediatric, or medical/surgical clinical experience. Prior home care or hospice experience preferred.
