Liberty Healthcare Management is currently looking for a passionate and experienced Registered Nurse to join our team as a Hospice Clinical Manager. In this role, you will be responsible for coordinating case management of patient services, supervising care staff, and ensuring high-quality care standards.

Responsibilities

• Coordinate case management of all patient services.
• Supervise assigned care staff and assist the Administrator as necessary.
• Provide direct patient care, including case management and Oasis care planning.
• Ensure efficient utilization of staffing resources and maintain accurate documentation.

Requirements

• Current RN licensure in North Carolina.
• Minimum of three years of clinical nursing experience, with hospice experience preferred.
• Strong knowledge of hospice/homecare regulations.
• Excellent organizational, communication, and critical thinking skills.
• Proficiency in computer applications and CPR certification.
