Join our team at UVM Health Network Home Health & Hospice as a Registered Nurse - Intake & Hospital Liaison. In this critical role, you will be part of the Intake and Referral team that serves as the hub of our operations, facilitating seamless transitions for patients into home health and hospice services.

Responsibilities

• Act as the primary contact for patients and referring providers, providing essential information and education about our services.
• Coordinate patient discharges, review clinical documentation, and assess patient eligibility for care services.
• Collaborate with clinicians and program leaders to ensure timely and effective care transitions.

Requirements

• Associate's degree in nursing and valid RN license.
• Minimum of two years of RN experience.
