Job Description
Join the Team Select Family in providing quality home health care to our valued patients and their loved ones. With more than 35 locations across 11 states nationwide, Team Select Home Care aims to provide exceptional in-home patient care with new methods of reducing re-hospitalizations and driving clinical outcomes.
Responsibilities
- 1:1 patient care.
- Working with pediatric/adult patients with g-tube, trach, and/or ventilator.
- Documenting electronically.
- Patient assessments.
- In-person.
- Developing meaningful long-term relationships.
Qualifications, Licenses, and Certifications
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
- Maintains a current CPR certification.
- BLS (Basic Life Support) Certification (preferred).
- Pediatric experience (preferred).
