Job Description
We are currently seeking a dedicated Social Worker to join LifeBridge Health's compassionate team. In this full-time position based in Baltimore, MD, at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and families facing various challenges. The role involves addressing issues such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on end-of-life matters.
Responsibilities
- Deliver clinical social work services to patients and families.
- Conduct psychosocial assessments to identify and address common challenges.
- Collaborate with healthcare teams to provide comprehensive support.
- Offer counseling and guidance on complex emotional and practical issues.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
