Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing a range of common concerns such as adjustment to institutional living, financial issues, family dynamics, medical understanding, aging-related changes, discharge planning, and counseling for end-of-life matters.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common patient and family concerns related to various life transitions.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred.
- LGSW certification required.
