Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. This is a full-time position with a day shift schedule. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, families, addressing various issues including adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments to address common problems.
- Assist with discharge planning and counseling on end-of-life issues.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
