Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. The ideal candidate will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying. This full-time position offers a day shift schedule from 8:00 am to 4:30 pm.
Responsibilities
- Conduct psychosocial assessments of patients/residents and families.
- Provide counseling and support for issues related to aging, family relationships, and medical treatments.
- Assist with discharge planning and empower patients to cope with life changes.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred; LGSW licensure required.
- Strong communication and interpersonal skills.
- Compassionate and empathetic attitude towards patient care.
