Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health, a renowned health care provider in Baltimore, MD. As a full-time member of our team, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues including adjustment to institutional living, financial concerns, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and address financial and family-related concerns.
- Collaborate with the interdisciplinary team to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C certification preferred.
- LGSW credential required.
