Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide valuable clinical social work services and psychosocial assessments to patients and their families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Conduct psychosocial assessments for patients/residents and families.
- Provide counseling on a range of topics including medical treatments, aging, discharge planning, and grief support.
- Collaborate with interdisciplinary teams to ensure holistic patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
