Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges they may face. This includes supporting individuals in adjusting to institutional living, resolving financial concerns, dealing with family relationships, and offering counseling related to medical diagnoses and end-of-life decisions.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist individuals in managing and coping with emotional and social challenges.
- Collaborate with healthcare professionals to develop and implement care plans.
- Participate in discharge planning and provide support for end-of-life care.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong communication and interpersonal skills.
