Job Description
We are looking for a Social Worker to join Northwest Kidney Centers in a full-time, variable position with flexible scheduling options. As a Social Worker, you will be responsible for providing social work intake, patient advocacy services, and coordinating social services for patients, families, and partners. You will play a key role in supporting the patient voice, promoting communication, and educating patients on their rights and grievance processes.
Working as part of the interdisciplinary team, you will conduct comprehensive social service assessments for each patient, develop individualized care plans, evaluate patient status, and implement treatment plans ensuring quality care. Additionally, you will participate in quality assessment performance improvement activities to enhance patient care.
Qualifications
- MSW degree
- Current WA Social Worker License
- Valid Washington Driver's license, reliable transportation, and car insurance
- Preferred: Previous medical social work experience
