Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. In this full-time role based in Baltimore, MD at Levindale Hospital, you will provide essential clinical social work services to patients/residents and families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling on end-of-life matters.
Responsibilities
- Provide clinical social work services to patients/residents and families.
- Assess and address common social and emotional challenges of patients.
- Offer guidance on financial concerns and family relationships.
- Conduct discharge planning and counsel patients on end-of-life issues.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred.
- LGSW certification required.
