Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As a full-time member of our team, you will provide clinical social work services and psychosocial assessments to patients and families, addressing a variety of challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life issues. If you are passionate about helping others navigate difficult situations, this role is for you.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments and develop care plans.
- Assist patients in coping with medical diagnoses and treatments.
- Facilitate discharge planning and bereavement support.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
