Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health, a leading healthcare provider in Maryland. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges. This is a full-time position based in Baltimore, MD, at Levindale Hospital, offering day-shift hours.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and end-of-life support.
- Address financial concerns and family dynamics impacting patient care.
- Collaborate with healthcare teams to ensure comprehensive patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
