As a Case Manager, you will enjoy a diverse array of clients and independence in managing your caseloads.

We are currently hiring individuals to join our Hennepin DD waiver team, working with adults and adolescents with Developmental Disabilities, requiring prior experience in this field.

Job Responsibilities and Duties:

  • Assessing areas where assistance is needed for persons served on different waivers.
  • Coordinating resources and monitoring service delivery.
  • Developing service plans and documenting client interactions.
  • Attending meetings and collaborating with interdisciplinary teams.

Salary:

Non-LSW wage: $49,000/year | LSW wage: $51,600/year (salaried for 40hrs/week)

Requirements:

Required Qualifications:

  • Graduate from an accredited college in social work, psychology, sociology, or related fields.
  • Experience as a social worker/case manager/care coordinator, or relevant education.
  • Valid driver's license, vehicle with insurance, and clear background checks.

Preferred Qualifications:

  • Experience in education or treatment of individuals with developmental disabilities.
  • LSW or higher licensure.
