INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position at Northwest Hospital in Randallstown, MD, offers a sign-on bonus potential of $15,000. As an Inpatient Case Manager, you will work with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role is crucial in promoting patient wellness, improving care outcomes, optimizing health services utilization, and reducing payment denials.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• BSN required from CCNE accredited schools.
• Prior experience in utilization review, clinical care coordination, or case management is a plus.
