Shifts: Full-time with Weekend Commitment, Day and Evening shifts (8:30am-7:00pm)
Sign-On Bonus Potential: $15,000
Summary
The Inpatient Case Manager, working closely with the clinical team and medical provider, is responsible for discharge planning coordination and intervention. The role focuses on promoting patient wellness, improving care outcomes, ensuring efficient use of health services, and minimizing payment denials for patients with complex health needs.
Responsibilities
- Provide discharge planning coordination and intervention.

- Minimize denials of payment for a patient population with complex health needs.
Requirements
- Bachelor's degree or equivalent professional knowledge.
- BSN required from CCNE accredited schools.
- Experience in a specialized field.
