At Sunrise Senior Living, we are currently seeking a Care Manager to provide top-quality care and services to residents in our assisted living and reminiscence neighborhoods. As a Care Manager, you will build meaningful relationships with seniors and their families, assist with daily living activities, and ensure individual care needs are met.

Responsibilities

• Build relationships with residents and families, provide assistance with daily activities, and attend to care needs.
• Notify management of changes in residents' conditions and recommend necessary adjustments.
• Participate in developing Individualized Service Plans and updates.
• Ensure residents' well-being and safety throughout the shift.
• Collaborate with the community team to maintain compliance with regulations and safety standards.

Qualifications

• Positive attitude and flexibility to perform various duties.
• Previous experience working with seniors preferred.
• Strong communication, organizational, and time management skills.
• Good judgment and decision-making abilities.
