Job Description

Superior HealthPlan is seeking a Care Manager RN for Foster Care to perform care management duties, assess member's health status, and coordinate medical and supporting services to promote quality, cost-effective care. This position involves education, coordination of services between healthcare providers and non-medical staff, case management, and participation in integrated team care management rounds.

Responsibilities

  • Assess member's health status, resource utilization, treatment plan, and services.
  • Develop a plan of care based on assessment with specific objectives and interventions.
  • Coordinate services between Primary Care Physician, specialists, medical providers, and non-medical staff.
  • Provide patient and provider education.
  • Facilitate member access to community-based services.
  • Monitor referrals and report related risk management quality concerns.
  • Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems.
  • Direct care to participating network providers.
  • Participate in case management committees and special projects.

Requirements

  • Graduate from an Accredited School of Nursing.
  • Bachelor’s degree in Nursing preferred.
  • 2+ years of clinical nursing experience and 1+ years of case management experience in a managed care setting.
  • Current state’s RN license.
