Care Manager RN - Foster Care

Job Description

Superior HealthPlan is seeking a Care Manager RN - Foster Care to perform care management duties, assess members' health status, and coordinate all aspects of medical and supporting services across the continuum of care for select members.

Responsibilities

• Assess the member's current health status, resource utilization, treatment plan, and services
• Develop care plans with specific objectives and interventions to meet member's needs and promote desired outcomes
• Coordinate services between healthcare providers and non-medical staff, as necessary
• Provide patient and provider education
• Facilitate member access to community-based services
• Monitor referrals made to support the members’ care management plan
• Identify and report related risk management quality concerns
• Enter and maintain assessments, authorizations, and clinical information into various medical management systems
• Participate in case management committees and special projects related to case management

Requirements

• Graduate from an Accredited School of Nursing
• Bachelor’s degree in Nursing preferred
• 2+ years of clinical nursing experience
• 1+ years of case management experience in a managed care setting
• Solid knowledge of utilization management principles and healthcare managed care
• Current state’s RN license
