Position Summary

The CSC Case Manager at PermiaCare will provide psychoeducation, psychosocial rehabilitation, and case management services to individuals enrolled in the Coordinated Specialty Care-First Episode Psychosis (CSC-FEP) program. Responsibilities include assisting individuals aged 15 to 30 with employment/education needs and mental health recovery goals with a focus on engaging with primary care physicians and coordinating support services.

Qualifications

Bachelor's degree in psychology, social work, or related field
1 year of experience in social services preferred
Texas Driver's License, QMHP certification within 6 months

Responsibilities

Provide direct services to individuals in the program
Develop and monitor individualized recovery plans
Coordinate integrated services and transportation
Maintain accurate documentation and confidentiality
