As a Care Manager, you will provide high-quality care and services to seniors in our assisted living and reminiscence neighborhoods. Your role involves building meaningful relationships, assisting with daily activities, and ensuring a warm and supportive environment for our residents.

Responsibilities

• Build relationships with seniors and their families, assisting with daily activities and personal care needs.
• Monitor residents' conditions and recommend necessary adjustments in care levels.
• Participate in developing Individualized Service Plans and ensure residents' well-being.
• Promote a safe and enriching environment for residents, including dining assistance and life skills activities.

Qualifications

• Dedication to serving seniors with excellent customer service skills.
• Positive attitude, flexibility, and teamwork abilities.
• High School diploma/GED and relevant certifications as per state requirements.
• Previous experience with seniors preferred.
• Effective communication, organizational, and decision-making skills.
