Post Permanency Coordinator

Job Description

We are looking for a dedicated Post Permanency Coordinator to join the team at Asociación Puertorriqueños en Marcha, Inc. This role plays a crucial part in ensuring compliance with adoption regulations and providing essential services to children, youth, and families involved in the adoption process.

Responsibilities

• Assess the physical condition of resource homes.
• Complete Post Permanency Services according to SWAN guidelines.
• Maintain up-to-date documentation and files as per state and county requirements.
• Offer training and social work services to program families.
• Attend meetings, conferences, and court sessions as necessary.

Requirements

• Bachelor's Degree in Social Work, Psychology, or related field.
• Minimum one year of experience in the adoption system or direct social service delivery.
• Familiarity with PA Chapter 3700 and Microsoft Office Suite.
