Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health in Baltimore County/City. This full-time position offers a competitive salary and potential sign-on bonus. As a crucial member of our team, you will be responsible for delivering professional and compassionate services to individuals in their chosen residence, assessing their psychosocial, environmental, and financial needs, and working collaboratively with patients, families, and interdisciplinary teams to develop and evaluate care plans.
Responsibilities
- Deliver services in alignment with our Mission Statement and S.P.I.R.I.T. Values.
- Assess and address patients' and families' psychosocial, environmental, and financial needs.
- Collaborate with patients, families, and teams to formulate and evaluate care plans.
- Serve as a consultant, educator, and expert practitioner.
- Ensure evidence-based practice and optimal patient/family care.
Requirements
- Master's Degree in Social Work from an accredited school.
- LCSW certification required.
- Strong interpersonal communication skills.
- Ability to maintain accurate documentation records.
