Job Description

We are looking for a passionate SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common challenges such as adjustment to institutional living, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common challenges faced by patients/residents and families.
• Offer counseling related to various issues including adjustment, financial concerns, relationships, and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
