Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and more. Your responsibilities will include discharge planning, counseling on end-of-life issues, and support in times of transition.

Requirements

  • Basic professional knowledge, with a Bachelor's degree or equivalent.
  • 1-3 years of experience in a related field.
  • LBSW/LMSW/LGPC certification.
