Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As part of the social work team, you will be responsible for providing clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling related to end-of-life issues.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and support patients' transition from care facilities.
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
