Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist in discharge planning and support patients in coping with medical diagnoses and procedures.
• Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Solid understanding of social work theory and practice.
