SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services to patients/residents and families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments for patients/residents.
• Provide counseling on a range of issues including adjustment, financial concerns, and discharge planning.
• Collaborate with healthcare teams to support patients and families.

Requirements

• Bachelor's degree; LBSW/LMSW/LGPC certification.
• 1-3 years of experience in social work.
• Strong knowledge of social work theory and practice.
