Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services to patients and families, addressing various psychosocial needs such as adjustment to institutional living, family relationships, and discharge planning.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address common challenges related to financial concerns, understanding medical procedures, aging, and end-of-life issues.
• Collaborate with interdisciplinary teams to develop and implement care plans.

Requirements

• Basic professional knowledge with a Bachelor's degree in social work.
• 1-3 years of related experience as a Social Worker.
• LBSW/LMSW/LGPC certification.
