SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Offer psychosocial assessments and counseling to patients/residents and families.
• Assist with discharge planning and provide support related to death and dying.
• Address various challenges faced by patients, such as financial concerns and family relationships.

Requirements

• Bachelor's degree with 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
• Compassionate and empathetic approach to patient care.
