Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment issues, financial concerns, and end-of-life counseling.

Responsibilities

• Offer clinical social work services to patients and families.
• Conduct psychosocial assessments and provide counseling.
• Assist with discharge planning and end-of-life support.
• Collaborate with healthcare teams to address patient needs.

Requirements

• Bachelor's degree and appropriate licensure (LBSW/LMSW/LGPC).
• 1-3 years of relevant experience in social work.
• Knowledge of social work theory and practice.
