Job Description
We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face.
Responsibilities
- Offer psychosocial assessments and support to patients and families.
- Assist with adjustment to institutional living and aging-related changes.
- Provide counseling on medical diagnoses, discharge planning, and end-of-life issues.
- Collaborate with healthcare teams to address patient needs.
Requirements
- Basic professional knowledge with a Bachelor's degree.
- 1-3 years of experience in social work.
- LBSW/LMSW/LGPC certification.
