Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. As a member of our Allied Health team at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients and their families. Your role will involve addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Provide counseling and support services to address emotional and social needs.
- Assist with discharge planning and coordination of care.
- Collaborate with interdisciplinary teams to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
- Strong interpersonal and communication skills.
