SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and experienced Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and families facing various challenges related to institutional living, medical issues, aging, discharge planning, and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to address patients' and families' emotional and social needs.
• Assist with financial concerns and help patients navigate complex healthcare systems.
• Collaborate with interdisciplinary teams to develop comprehensive care plans.
• Offer support and guidance to individuals coping with changes and loss.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed by the state as LBSW, LMSW, or LGPC.
• Demonstrated knowledge of social work theory and practices.
