SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face in a hospital setting. You will play a crucial role in supporting individuals and families through difficult situations, such as adjustment to institutional living, financial concerns, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common challenges including adjustment to institutional living, financial concerns, and discharge planning.
• Offer support and counseling on issues related to medical procedures, aging, and end-of-life care.
• Collaborate with healthcare teams to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of experience in a healthcare setting.
• Licensed as LBSW, LMSW, or LGPC.
• Knowledge of clinical social work practices and ethical standards.
