SOCIAL WORKER PRN

Job Description

We are looking for a Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address common challenges including adjustment to institutional living, financial concerns, and family relationships.
• Provide counseling related to death and dying, discharge planning, and aging.

Requirements

• Bachelor's degree or equivalent.
• 1-3 years of experience in a related field.
• LBSW/LMSW/LGPC certification.
