Job Description
We are looking for a dedicated PRN Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, focusing on various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist in discharge planning and addressing patient concerns.
- Collaborate with healthcare teams to ensure comprehensive care.
- Support patients in coping with medical conditions and aging-related changes.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong communication and interpersonal skills.
