Job Description

We are looking for a passionate SOCIAL WORKER PRN to join our dedicated team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services and psychosocial assessments to patients and families, addressing common concerns such as adjustment to institutional living, financial issues, family relationships, and counseling related to various life changes.

Responsibilities

• Conduct psychosocial assessments and develop care plans.
• Assist patients and families in coping with emotional and psychological challenges.
• Coordinate with interdisciplinary teams for comprehensive patient care.
• Provide counseling and support for end-of-life care and bereavement.

Requirements

• Bachelor's degree with LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in social work.
• Strong understanding of psychosocial theory and practice.
• Excellent communication and interpersonal skills.
